Below are the Document and Form Requirements For All Discovery Christian School Students (PDF's)
We will receive most of these for new students transferring in after we send the Records Release to the prior school. We will have parents fill this out at student interview.
Kindergarten Students:
● Physical Exam and Immunization Record or Religious Exemption Form
● Certified Birth Certificate (not the one from the hospital)
Second Grade Students:
Sixth Grade Students:

(Sports Physicals Do Not Qualify)
Ninth Grade Students:
Physical Exam and Immunization Record or Religious Exemption Form
(Sports Physicals Do Not Qualify)
